Welcome to Your Patient-Centered Medical Home!

Azalea Health is dedicated to providing care that is accessible, consistent, coordinated, collaborative and compassionate.

What is a Patient-Centered Medical/Dental Home?

A patient-centered medical/dental home places you at the center of care, working with your health care team to create a personalized plan for reaching your healthcare goals. Your primary care team is focused on getting to know you and earning your trust; we care about you while caring for you. Azalea Health has adopted technology that makes it easier for you to get health care when and how you need it. You can reach your doctor or care team through email and after-hours phone calls. Our patient portal will help you stay on top of your health and medical history.

We offer personalized care plans that you help design that address:

  • Health concerns
  • Medication review to help you understand and monitor the prescriptions you are taking
  • Coaching and advice to help you follow your care plan and meet your health care goals
  • Referral services to specialists and community resources

Wherever your journey takes you, your care team will help guide and coordinate your care.
