Mammogram Screening Events

• Wednesday, December 30, 2020 8 a.m. to 12 p.m. at our Interlachen location (1213 State Road 20) – call (386) 326-7389 to schedule a mammogram screening on this date

We care about your health!

We will be holding preventive screening events in collaboration with Precision Imaging and the Putnam County Health Department (PCHD) on Wednesday, December 16th and Wednesday, December 30th.

Precision Imaging will be onsite with their mobile mammogram bus to provide easy, convenient and life-saving screenings for breast cancer. Most insurances are accepted; however, Precision does not accept Medicaid or Humana on the mobile unit. A reduced rate of $150.00 for a 3D mammogram will be offered if you are uninsured and do not qualify for a free screening mammogram through the Florida Breast and Cervical Cancer Early Detection Program. The FBCCEDP application should be submitted in advance and is available at all Aza Health medical offices. The PCHD will be on-site to complete applications during the events.

Aza Health will also be offering $5.00 Cervical Cancer screenings during these events.
